This means that your insurer is trying to withdraw the insurance premium amount from your bank account, but does not have enough money to cover the insurance premium, therefore:

1. Your bank charges a fee.

2. You lost your insurance.

NSF = Insufficient nodes.

INS PREM is probably an insurance premium.

ACH = bank transfer.

It looks like you have set up automatic payments with your bank account and there is not enough money in the account. Your bank will charge you for the refusal to pay and your insurance company may also charge you.

On the other hand, I think most banks now pay for this thing and charge you even though they charge you. So make sure you know if the payment was rejected or paid.
